29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) On or about September 26, 2024, located at 74th Street and Ocean Terrace, Miami Beach, Florida, the employer did not ensure each employee who operated a CAT TL1055C All-Terrain telehandler was evaluated at least every three years. The employees' last evaluation occurred on 6/29/2021.
