29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, 2009 Edition including revision one dated May 2012 and revision 2 dated May 2012: a) W. Shelley Dr./Darley Rd., Claymont, DE 19703: On or about September 24, 2024, employees were exposed to struck by hazards when they worked in live lanes of traffic and stopped vehicular traffic using hand signals alone, employees were not using the required slow/stop paddle as required in the MUTCD 2009 edition 6E .07. b) W. Shelley Dr./Darley Rd., Claymont, DE 19703: On or about September 24, 2024, employees were exposed to struck by hazards when they stood in active roadways with her back turned to traffic and failed to follow flagger procedures in section 6E .07 of the 2009 Edition of the MUTCD. The AWP, Inc dba Area Wide Protective was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.201(a), which was contained in OSHA inspection number 1736165, citation number 1, item number 4 and was affirmed as a final order on 6-3-2024, with respect to a workplace located at 5619 Wilson Mills Rd. Highland Heights, Ohio 44143.
