At the worksite, located at 1037 Professional Park Dr Brandon, FL 33511; On or about 9/17/24, the employer exposed employees to fall hazards of approximately 9ft, in that, extension ladders were not extended for employees 3ft above the work surface while conducting patch work on a commercial building. b.)
