29 CFR 1910.119(j)(4)(iv): Documentation of each inspection and test that had been performed on process equipment did not identify the date of the inspection or test, the name of the person who performed the inspection or test, the serial number or other identifier of the equipment on which the inspection or test was performed, a description of the inspection or test performed, and the results of the inspection or test: Ammonia Refrigeration Machine (Engine) Room: Documentation of each inspection and test that had been performed on components of the piping system of the employer's ammonia refrigeration system did not include the following information: a) On or about September 3, 2024, documents of each inspection and test performed for a drain valve and drain plug for a recirculating pump for the low pressure receiver (LPR) vessel did not include all information listed in ANSI/IIAR (American National Safety Institute/International Institute of Ammonia Refrigeration) IIAR 6 2019 edition 5.1.1.3 including, but not limited to, serial number or other identifier of the equipment on which the inspection or test was performed. b) On or about November 17, 2024, documents of each inspection and test performed for a purge line pipe for the low pressure receiver (LPR) vessel did not include all information listed in ANSI/IIAR (American National Safety Institute/International Institute of Ammonia Refrigeration) IIAR 6 2019 edition 5.1.1.3 including, but not limited to, serial number or other identifier of the equipment on which the inspection or test was performed.
