29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. On or about September 5, 2024, and at times prior thereto, on the 5th floor of the batch house, a hole in the walking/working surface was not provided with a cover or other means of protecting employees walking/working in the batch house from tripping into the hole.
