29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). a) In the warehouse, located at 5682 East Ponce De Leon Ave., Stone Mountain, GA 30083: On or about August 29, 2024, and at times prior, employees operated forklifts that had defects such as but not limited to a broken seatbelt, completely torn/ripped seat cushions, and broken propane toggle clamps, thereby exposing employees to struck and crushed by hazards.
