29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: (a) On or about August 26, 2024, at or near the intersection of S. Holly St and E. Arapahoe Road, the employer did not ensure that employees conducting concrete sidewalk removal activities were provided instruction in the recognition and avoidance of struck-by hazards involving spotter safety training, safe backing operations, and internal traffic control procedures while heavy equipment or trucks were performing backing operations at the job site. Employees were also stopping and signaling traffic to allow construction equipment to enter and exit the work zone without adequate training. This condition exposed employees to crushing and struck-by hazards while stopping traffic and backing dump trucks within the job site.
