29 CFR 1928.57(c)(5)(i)(A): Application of electrical power from a location not under the immediate and exclusive control of the employee(s) maintaining or servicing equipment was not prevented by providing an exclusive, positive locking means on the main switch: On or about August 14, 2024, at a grain storage silo located at 11682 County Road 691 in Dudley, Missouri, employees were exposed to 480-volts of electricity. The employer failed to provide a means for employees to control or lock a main electrical switch in the off position to deenergize electrical lines prior to the installation of a 5 hp, Tech Top Industrial Electric Motor. An employee suffered fatal injuries when contact was made with the electricity.
