29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about August 7, 2024, at the worksite, 610 North Madison Street, Rome, NY 13440: Employees engaged in residential roofing activities were using a portable extension ladder to access the roof. The employer failed to ensure the side rails of the ladder were extended 3 feet above the access point to the roof.
