29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a worksite located at 1009 Wilds Parkway, Greenwood, MO, employees were exposed to fall hazards. The employer exposed the employees to these hazards by allowing them to use a portable extension ladder to access an upper landing surface when the side rails of the ladder did not extend at least 3 feet above the level to which it was being used to gain access, nor was the ladder secured to the structure with a grasping handle available for employee use.
