29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements. On or about August 14, 2024, employees engaged in distribution operations in the commercial bakery facility located at 4651 F Street, Omaha, Nebraska 68117 were exposed to slips, trips, and fall hazards in that employees were using a stair system for access and egress from the facility (next to door #1) and the stair system was missing a handrail on the enclosed side.
