29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole (including skylights) that was 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems: (a) Roof; On or about August 2, 2024, and dates prior, employees were not protected from falling through holes on bins by a guardrail system, travel restraint system, or personal fall arrest system when bin covers and grating are removed to check inventory levels with a laser distance meter and while shoveling wet/spoiled feed into them.
