29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a)Form Tight Inc. at 11600 East 51st Avenue, Denver, CO 80239: On or about and at times prior to July 24, 2024, the employer did not ensure that employees operating the electric, LPG, and stand-up forklifts without the truck operators' performance being evaluated at least once every three years. This condition exposed employees to struck-by hazards associated with improper forklift operations.
