29 CFR 1910.28(b)(11)(ii) as required by 29 CFR 1960.8(b): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements: (a) Basement loading dock; On or about July 9, 2024, and dates prior, a stairway with 6 treads, 7 risers, and less than 44 inches wide with two open sides, was used by employees to access the loading dock and other areas of the basement and was not equipped with one stair rail system on each open side.
