29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment. a. On or about July 2, 2024, on the production floor, personal lockout or tagout devices were not attached to a group lockout device when employees were performing service and/or maintenance, to include cleaning, the Line 6 Stringline Proofer, Winkler Model #681979. Employees were thereby exposed to the hazards associated with re-energization of equipment.
