29 CFR 1926.502(i)(4):All covers were not color coded or marked with the word "HOLE" or "COVER" to provide warning of the hazard: a) At 3062 SR 143, Elmore AL: On or about June 25, 2024 and at times prior thereto; the employer exposed employees to fall hazards of approximately 26 feet and 6 inches to the concrete below in that employees were performing roofing activities around a hole for which the cover was not appropriately marked or color coded to provide warning of the hazard.
