29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: On or about 06/17/2024, employees were exposed to caught-in-between hazards while working at a manufacturing facility located at 5490 County Road 33 in Koshkonong, MO. An employee was removing debris from the inside of a material mixer chute which was not turned off or shutdown allowing the employee to enter the danger zone with live parts in operation.
