29 CFR 1926.251(a)(1): Defective rigging equipment was not removed from service: a) Jobsite located at 3615-35 Chestnut St. Philadelphia, PA 19104 - On or about June 17, 2024, the employer did not ensure defective Doka Framax Lifting Hooks were removed from service. Employees received a load of preassembled Doka Framax formwork hoisted using defective Doka Framax Lifting Hooks from a tower crane, thereby exposing employees to struck by hazards.
