29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a.) 26 East Chestnut Street, Lancaster, PA - Chestnut Street side - On or about June 9, 2024, Employees were exposed to a fall of approximately 15 feet through an open ventilation shaft pit while transversing through an exit door discharge that lead to an open ventilation shaft/pit. b.) 26 East Chestnut Street, Lancaster, PA - Chestnut Street side - On or about June 9, 2024, hotel maintenance staff were tasked to remove ladders from through a ventilation shaft/pit without means of fall protection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
