29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) LHJ Remodeling, LLC located at 7507 Walnut Dr, CANAL WINCHESTER, OH 43110: On or about June 3, 2024, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards. Roofers were removing/installing shingles on a roof that varied from over ten to 22 feet with the employees working at different levels. The employees were not utilizing compliant fall protection which exposed them to falls of 22 feet. LHJ Remodeling, LLC was previously cited for a serious violation of this Occupational Safety and Health standard 1926.21(b)(2), which was contained in OSHA inspection number 1448221, citation number 1, item number 2 and was affirmed as a final order on February 26, 2020, with respect to a workplace located at 297 Bruce Ct., Westerville, OH 43081.
