29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary: a. At 1215 Imeson Park Blvd in Jacksonville Florida. On or about June 4, 2024, in the 12-foot deep trench, the employer exposed employees to crushing, struck-by, and engulfment hazards from spoil piles that were placed on the top of the leading edge of the trench sidewalls without any setback distance.
