29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about May 23, 2024, the employer failed to ensure that the N5 milk carton machine line operators were trained as authorized to apply lock out tag out when clearing jams on the machines. Employees are clearing jams regularly when the machine is in operation and the machine is not being locked and tagged out prior to clearing a jam.
