29 CFR 1926.1203(e)(2)(iii): Before an employee enters the space, the internal atmosphere must be tested, with a calibrated direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order. Any employee who enters the space, or that employee's authorized representative, must be provided an opportunity to observe the pre-entry testing required by this paragraph. At the worksite located at 1601 SW MO-150 HWY Lee's Summit, MO: On May 11, 2024, employees were exposed to inhalation hazards in the form of carbon monoxide. An employee entered a permit required confined space without prior atmospheric testing. Before an employee entered the space, the internal atmosphere was not tested as required and/or any employee who entered the space, or that employee's authorized representative, was not provided an opportunity to observe the pre-entry testing required by this paragraph.
