29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a) On or about April 23, 2024, in the Engine Room #1 at Americold Logistics LLC Samuel facility, employees were exposed to fire, explosion and toxic gas release hazards when the employer did not replace or recertify the pressure relief valves on oil pot of Vessel V-1 (High Temp Accumulator) every five years according to their adopted RAGAGEP, IIAR Bulletin 110 (Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems) Edition 1993, Section 6.6.3. b) On or about April 23, 2024, in the Engine Room #1 at Americold Logistics LLC Samuel facility, employees were exposed to fire, explosion and toxic gas release hazards when the employer did not replace or recertify the pressure relief valves on oil pot of Vessel V-2 (Low Temp Accumulator) every five years according to their adopted RAGAGEP, IIAR Bulletin 110 (Start-up, Inspection and Maintenance of Ammonia Mechanical Refrigerating Systems) Edition 1993, Section 6.6.3. c) On or about April 23, 2024, in the Engine Room #1 at Americold Logistics LLC Samuel facility, employees were exposed to fire, explosion and toxic gas release hazards when the employer did not ensure the ammonia refrigeration piping and system components; including but not limited to the relief valve vent header, relief valves discharge pipes were properly identified and labelled according to their adopted RAGAGEP, IIAR Bulletin No. 109 (Minimum Safety Criteria for a Safe Ammonia Refrigeration System) Edition 1997, Section 4.7.6.
