29 CFR 1910.333(b)(2)(iv): Verification of deenergized condition. The requirements of this paragraph shall be met before any circuits or equipment can be considered and worked as deenergized. On or about April 1, 2024, at a jobsite located at 1919 South Broadway Street, in Green Bay, Wisconsin; in Building 141 on the second floor MCC Room, verification of de-energization of the 4160V switchgear on the load side and line side was not conducted prior to pulling wires for the Boiler 10 project exposing employees to electrical shock hazards.
