29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached: A). On or about 03/22/2024, at the site of 70-23 Juno Street. Forest Hills, NY Two employees were connected to one anchorage point with their personal fall arrest system while working on a pitched roof approximately 30-feet in height. Five anchorage points were installed for a crew of seven employees who were working on the roof.
