29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. On or about March 18, 2024, employees were exposed to struck-by hazards at or near 5610 Wilson Mills Rd., Highland Heights, OH, 44143 while performing an incorrect flagging zone on a multilane highway, where the adjacent roadway did not have adequate traffic control devices in accordance with the MUTCD part 6, to provide protection for traffic and workers and where the employer failed to provide employees with a traffic control plan or typical application.
