29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: On or about March 14, 2024, and times prior thereto the employer entered and or broke the plane of entry at frac tanks and vacuum truck tanks to conducted cleaning operations without a written PPE certification of assessment of potential hazards. As a result, the employer did not verify by written certification that a workplace hazard assessment to determine if goggles, face shields, respirators, protective suits, rubber gloves or rubber boots were needed.
