29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including but not limited to, the following: Specifying acceptable entry conditions; Isolating the permit space; a. Hassia Auto-Stacker - On or about January 26, 2024, the employees engaged in changing the motor from Goldco Industries Model H3060 auto stacker lift #1 to #2 entered into the permit required confined spaced without having procedures or practices in place to safely enter the space and isolate the permit space to minimize the hazardous conditions entrants would face. Advanced Food Products LLC was previously cited for a violation of this occupational safety and health standard 1910.146(d)(3), which was contained in OSHA inspection number 1601440, citation number 1, item number 4a and was affirmed as a final order on 12/21/2022, with respect to a workplace located at 158 West Jackson Street, New Holland, PA 17557. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
