29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. At a facility located at 127 Claxton avenue, Carthage, MO 64836, employees were exposed to slip, trip and fall hazards. The employer exposed the employees to these hazards when employees worked near an unguarded hole in the walking surface of the catwalk near the AST palletizer area.
