29 CFR 1910.147(c)(7)(i)(A):Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. (a) On or about January 19, 2024 the employer did not ensure that employees who perform service and maintenance tasks on the Totani Pouch Machine were trained on the means and methods to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up, energization and/or release of stored energy which could result in caught-in, crushing and amputation hazards from the heat stomp or in-going nip points created by the rollers when feeding the zipper through the zipper seal station. (b) On or about January 30, 2024, the employer did not ensure that employees who perform service and maintenance tasks on the Totani Pouch Machine were trained on the means and methods to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up, energization and/or release of stored energy which could result in caught-on, crushing and amputation hazards from the heat stomp or in-going nip points created by the rollers while performing set up tasks for production. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
