29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) a) Laydown area: On or about January 23, 2024, and times prior, the employer exposed employees to Crushed-by hazards, in that the employer did not remove from service the Toyota PIT, model 7FDKU40, serial number 60926 which was missing the left side door, which was used by employees to move various containers to and from the laydown area and inside the building. b) Laydown area: On or about January 23, 2024, and times prior, the employer exposed employees to Crushed-by hazards, in that the employer did not remove from service the Toyota PIT, model 7FDKU40, serial number 60926 which had a faulty seatbelt that did not secure/latch when employees operated the forklift.
