1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a. At the rebar manufacturing shop, on or about January 17, 2024, the employer exposed employees to amputation and crushing injuries from the unexpected release of hazardous energy, in that the employer had not performed periodic reviews at least annually of the written energy control procedures that outlined the scope, rules, and steps for the control of hazardous energy for employees required to perform maintenance and service on RMS Hydraulic Shearline, Model 611, S/N: 306, the RMS Rebar Bender Machine, Model 14, S/N: 14-M-401, and the RMS Radial Bender, S/N: 229 industrial machine.
