29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about January 17, 2024, and at times prior thereto, an employee was working beneath the catwalk without head protection while another employee on the upper level of the catwalk operated a nail gun, exposing the employee to struck-by hazards.
