Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(6)(i):The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: (a) On or about January 2, 2024, and at times prior, the employer did not evaluate the lockout/tag out (LO/TO) program and procedures associated with de-energizing the MAC Process Model MD40 Airlock to remove a 55-gallon drum . The employer authorized using the on/off controls and bump procedures to remove jams exposing employees to caught-in and amputation hazards. An employee suffered multiple finger amputations on the right hand when reaching into the airlock system attempting to remove a jam while a supervisor was operating the controls.
