29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: Non-ferrous Metal Room Instance 1: On or about December 20, 2023, an employee's fingertip was amputated while operating an alligator shear that did not have the extension table extended keeping the employee's hands out of the danger zone. Instance 2: On or about December 20, 2023, a foot pedal controlling the power to an alligator shear was not adequately covered to prevent employees from accidently stepping.
