29 CFR 1910.147(d)(4)(i):Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. On or about December 19th, 2023, an employee engaged in commercial manufacturing activities, on the production floor of the facility located at 6161 Abbott Drive, Omaha, NE 68110, were exposed to amputation hazards in that employees were conducting maintenance at the point of operation on the YLM CNC Tube Bending Machine Number TB1 without applying a lock and or tag as means of isolating hazardous energy sources and preventing unintentional restart of the equipment.
