29 CFR 1910.147(f)(3)(ii): Group lockout or tagout devices were not used in accordance with the procedures required by paragraph (c)(4) of this section including, but not necessarily limited to, the following specific requirements: On or about January 24, 2024, in the induction area, employees who replace tape on the taper machine, after it has been locked out by maintenance, do not have their own locks and/or tags to place on the energy sources.
