29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers was equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements. a) Production area lower level to upper level: The employer did not ensure that the stairs leading from the lower production area to the upper production area had standard hand rails on each side in accordance with table D-2. The stairs had four risers and did not have any standard hand rails, on or about 12/6/23.
