29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about December 5, 2023, and at times prior, on the west side of the residence located at 850 San Pedro Avenue, Coral Gables, Florida, an employee was exposed to a 21-foot fall hazard while using an extension ladder which did not extend at least 3 feet beyond the upper landing when accessing a metal roof.
