29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about 11/23/2023 at the southeast corner of the roof; employees were replacing the roof of a repurposed church building with varying roof pitches, with the main roof having a 10 in 12 pitch. An employee, wearing a personal fall arrest harness but not tied off, was on the roof holding a 4'x 8' sheet of plywood when a gust of wind caught the board, the employee lost his balance and suffered a fatal fall of approximately 50 feet. NO ABATEMENT CERTIFICATION REQUIRED
