Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.178(l)(3):Training program content. Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace. (l)(3)(i)(G) Fork and attachment adaptation, operation, and use limitations; (l)(3)(ii) Workplace-related topics: (l)(3)(ii)(C) Load manipulation, stacking, and unstacking; (l)(3)(ii)(I) Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. a) On or about November 2, 2023, the employer did not adequately train powered industrial truck operators in topics to include but not limited to (l)(3)(i)(G), (l)(3)(ii), (l)(3)(ii)(C), (l)(3)(ii)(I) on the safe operation and handling of the Galbreath self-dumping hopper, in that employees were not trained on the proper procedures of how to manipulate the hopper if it does not self dump. On or about November 2, 2023 employees were exposed to a caught in hazard from the hopper tipping while trying to force the hopper to tip by getting off the forklift and using a hammer to strike the hopper.
