29 CFR 1926.1053(b)(1): When portable ladders had been used for access to an upper landing surface, the ladder side rails had not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder had been used to gain access. On or about October 27, 2023, R Garcia LLC did not ensure that a portable ladder used to access the roof was extended at least 3 feet above the upper landing surface. The ladder extended approximately 1 foot above the upper landing surface in a worksite at 201 Crystal Crossing Lane in Wentzville, Missouri, which exposed employees to fall hazards.
