29 CFR 1926.1204(c)(6): Employees in manholes were not protected from struck by or overhead hazards caused by unsecured tools, materials, and equipment when the cover was removed and the opening was not immediately guarded by a railing, temporary cover, or other temporary barrier. a) Greenwich Drive Feeds Into Pacific Boulevard, West Windsor NJ: On 10/12/23 a company employee and the company worksite Foreman were exposed to struck by hazards falling up to and in excess of 10 feet 2 inches onto them, when working in manholes that were not provided with covers, railings, etc. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILUE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
