Mingo's Construction LLC located at 860 Jaeger Street, Columbus, OH 43206: On or about October 4, 2023, during the construction of a mixed-use (commercial/residential) building, the employer did not ensure that rigging equipment for material handling was inspected prior to use on each shift and as necessary during its use to ensure that it was safe and free of defects. The following condition exposed employees to struck-by hazards.
