29 CFR 1910.178(l)(6):The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: At a job site located at 160 JD Power Trail, Jefferson City, MO; An employee encountered a fall from a work platform forklift attachment which resulted in an in-patient hospitalization. The fall hazards encountered were on or around September 22, 2023. The employer did not ensure that each forklift operator was certified to use a forklift and employees were not evaluated to ensure competency to operate a powered industrial truck safely.
