29 CFR 1910.244(a)(2)(i): In the absence of a firm foundation, the base of the jack was not blocked: a. On or about September 27, 2023, at Locomotive Railserve 8216 Mod GP-9 at the USD terminal Track A spur, 719 Richard James Avenue, Stroud, OK. 74079 , a bottle jack base was not blocked and lacked a firm foundation when it sat on a railroad tie and supported locomotive traction control motor #4. An employee worked under locomotive traction control motor #4 was exposed to crush-by hazards.
