29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked, or limited in height so that it was stable and secure against sliding and collapse: a) On or about 09/21/2023; in the shop, employees were exposed to struck-by hazards as tires, weighing approximately 75 lbs., were stored on racks at a height of around 15 feet without securing them against sliding. An employee was fatally injured when a tire that was stored vertically on its tread, on top of a small rim, was accidentally hit by the movement of a ladder, causing the tire to roll off the rack and strike the employee.
