29 CFR 1910.146(k)(3): Whenever an authorized entrant entered a permit space to perform rescue services, retrieval systems or methods were not used to facilitate non-entry rescue: a) Boiler Room- On or about July 6, 2023, employees performing maintenance tasks were exposed to confined space hazards without the use of a retrieval line or harness during a permit-required confined space entry into Boilers #1 and #2. l) Super Hog Conveyor Pit- On or about September 21, 2023, employees performing maintenance tasks were exposed to amputation hazards from entry into a permit-required confined space with a running conveyor and woodchipper without the use of a mechanical retrieval device, lifeline, or harness.
