29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 3, 2023, during the installation of roofing material at a job site located at 85 Montrose W. Ave, Copley Ohio 44321; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface. Flex Star Certified Roofing Installers Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1638706, citation number 1, item number 2 and was affirmed as a final order on (2/2/2023), with respect to a workplace located at 637 N. Jackson St, Lima Ohio 45801.
